Please use this form for new players and re-registering existing players for the 2021/22 season:

The form can be downloaded and filled in digitally on a computer and then printed off and signed. This will help to make all details clearer and the registration process quicker.
Alternatively it can also be printed off and filled in by hand, but we ask all forms to be completed as clearly as possible.

All parts of the form are completed clearly.

All parents/guardians sign the consent and all players sign (or write their name in their handwriting) in the appropriate space agreeing to observe the Club Code of Conduct and the Rules and Regulations of the Football Association, Surrey County FA and all competitions in which the Club participates.

All registrations are complete and supplied with x3 passport sized photos (1 of which should already be attached to the form in the appropriate space).

FOR NEW REGISTRATIONS ONLY please include a copy of the player's passport or birth certificate in order for the player to be registered with the League.
Season 2021/2022...
The fee for the season 2021/22 remains at £180. Incredible value! DO NOT INCLUDE ANY MONEY WITH THIS FORM. An invoice will be emailed to you directly with the payment details.
Players will not be able to play until the full fee is paid (unless otherwise agreed with the Club).

Photos of the completed form, a headshot photo (with a plain background) and player ID can be sent to the team manger via WhatsApp.
